A PT or OT at Commonwealth Community Care provides physical or occupational therapy services to patients in their home, work, or clinic setting at the request of the primary care providers. In addition to providing direct clinical care, the PT provides consultation to the Primary Care Team around equipment requests, policies and procedures. The PT/OT also provides on-going assessment and feedback to the Rehabilitation Services Manager of the rehabilitation needs of the patients. The PT/OT works collaboratively with other team members and community providers to ensure appropriate and comprehensive attention to the rehabilitation needs of patients.
-Performs an initial comprehensive evaluation and home assessment of assigned patients.
-Develops and implements a comprehensive plan of care in collaboration with the patient, caregivers and primary care team that addresses the clinical, education, psycho-social, and support needs of the patient and family. Meets expectations of a minimum of 10 visits per week.
-Ensures that all recommended and relevant services are provided to the patient in a timely manner.
-Evaluates episodic injuries and illnesses.
-Provides periodic re-evaluations of functional status.
-Recommends and facilitates community resources to meet the needs of the patients as appropriate.
-Orders appropriate equipment in coordination with the primary care team.
-Participates in seating clinics as appropriate to facilitate and coordinate the service needs of the patient.
-Facilitates and/or participates in patient care conferences.
-Reviews cases with the primary care team on a regular basis.
-Communicates with regulating agencies that may be involved in care.
-Works with staff of community based organizations, including residential and day program staff, providing consultation and education necessary to achieve maximum patient outcomes.
-Advocates for patients in a culturally competent manner.
-Ensures that patients’ right to fair and equitable treatment and self-determination are well established and maintained.
-Seeks maximum patient and family participation and independence.
-Provides telephone triage for clinical concerns of patients, care providers, family and significant others.
-Provides coverage for other therapists as necessary.

-Bachelor of Science degree or above in physical therapy
-Current Massachusetts license as a physical or occupational therapist
-RESNA Certified ATP preferred
-Minimum of two years clinical experience with a focus on home care, disability, customized seating and rehabilitation equipment
-Demonstrated experience collecting and synthesizing third party research through publically available sources
